Abstract
It has long been recognized that the liver plays an important role in carbohydrate homeostasis.
Petrides reported that Diabetes mellitus in insulin resistant cirrhotic patients developed as the result of progressive impairment in insulin secretion with the development of hepatic insulin resistance leading to fasting hyperglycemia and a
diabetic
glucose tolerance profile.
Diabetic patients with debilitating iLLness, including hepatic cirrhosis and acute alcoholic heptitis, appear to be more prone to infections, and the accumulating evidence for an immunological defect superimposed upon the metabolic abnormalities
of
diabetes is becoming increasingly convincing.
We described a patient with diabetes mellitus and liver cirrhosis who has purpura on body due to falling down, resulting in perineal and scrotal necrosis(Fournier's gangrene) as well as foot gangrene. Despite aggressive antibiotic therpy and
debridement, Fournier's gangrens is associated with a high motality .
We performed necrotic skin resection and reconstrucitve procedure with massive antibiotics therapy.
